Christmas Toy Request Form

PLEASE FILL OUT A SEPARATE FORM FOR EACH CHILD. Sign up here to be put on the list for a small Christmas toy from the Wal-Mart and Alliance Police Department partnership. Please fill out the form completely. Toys may or may not be available based on the number of donations made at the Alliance Wal-Mart. Recipients will be chosen based on the order in which they are received to the best of our ability. You may request a toy for any child - they do not have to be your own. Limited to addresses with 44601 zip code. REQUEST ARE DUE BY DECEMBER 16TH AT 5PM. Have a safe holiday season!

TOYS WILL BE DELIVERED TO THE HOME BY AN OFFICER ON FRIDAY, DECEMBER 23RD.

CHILDREN WHO DID NOT RECEIVE GIFTS FROM THE FIRE DEPARTMENT OR SALVATION ARMY WILL GET TOYS FIRST. ALL OTHERS WILL GET TOYS FROM THE POLICE DEPARTMENT ONLY IF SUPPLIES ARE STILL AVAILABLE.

Please provide some ideas that the child might like. We cannot guarantee that you will receive a toy similar to your request, but we will try if funding is available. Please keep the toy under the $20 price range.

Did you or someone else sign the child up for a toy from either the Fire Department or the Salvation Army. Checking yes does not automatically prevent you from receiving one from us.

Please provide us with any other additional information regarding your request such as any special needs of the child, why you feel this child should be considered, any pertinent information we need for delivery. Please let us know what they like to play with.
